    Records and assessment

    Photographs, a scan or impressions, and X-rays including a full jaw view. This is where it is decided whether your case suits aligners, braces, or neither.

    The plan, explained before you commit

    You are shown what will move, roughly how long it will take, whether any teeth need removing to make room, and what the result should look like. Nothing is fitted at this appointment.

  3. 360 minutes

    Fitting the appliance

    Brackets bonded to the teeth and the first wire placed, or the first set of aligner trays issued with attachments bonded where the plan calls for them.

  4. 420 minutes

    Reviews while the teeth move

    Short appointments every four to eight weeks to change a wire or check progress against the plan. This is the long middle of the treatment and it is mostly uneventful.

  5. 545 minutes

    Debond and retainers

    The appliance comes off, the teeth are polished, and retainers are fitted the same day. Teeth move back if they are not retained, so this step is not optional.

The plan matters more than the appliance

Every clinic in Koramangala can bond a bracket to a tooth. Very few things in dentistry are as easy to start badly and as slow to correct.

Teeth move because the bone around them remodels under steady, gentle pressure. Push them somewhere the bone cannot follow and the result is straight teeth sitting in receded gum with roots closer to the surface than they should be. That is not a rare horror story, it is the predictable outcome of moving teeth without first knowing where the roots and the bone actually are.

Which is why nothing is fitted at your first appointment here. The first appointment takes photographs, a scan or impressions, and X-rays including a full jaw view, and the plan is made from those. You are shown what will move, how far, roughly how long it will take, and whether space has to be created before it can happen. Then you decide.

The three options, and who each one suits

Metal braces. Stainless steel brackets and a wire. Still the most predictable appliance there is, still the least expensive, and still the only sensible answer for the genuinely difficult movements: rotating a round premolar, moving a root bodily through bone, closing a large extraction space. They are visible. For a lot of people that is the only thing wrong with them.

Ceramic braces. The same mechanics with tooth-coloured brackets. Far less obvious at conversational distance, slightly more fragile, slightly slower, and more expensive than metal. The usual choice for adults who need fixed braces and work in a job with a lot of meetings.

Clear aligners. A series of custom trays, each moving the teeth a fraction of a millimetre, changed every one to two weeks. Removable, so you eat and brush normally and there is no white-spot risk around a bracket. Excellent for mild to moderate crowding, spacing and relapse after earlier treatment. Less predictable for complex movements, and entirely dependent on you actually wearing them.

That last point is worth expanding, because it is where aligner treatment succeeds or fails. They work twenty two hours a day or they do not work. Two hours out is a meal and brushing your teeth. An aligner sitting in a drawer moves nothing, and a patient who has under-worn for a month arrives at a review where the next tray does not seat and the plan has to be restarted.

What we look for before saying yes

The assessment is not only about how the teeth look.

Gum health comes first. Moving teeth through inflamed gum with existing bone loss accelerates that loss. If there are pockets, they get treated and re-charted before any appliance goes on, not in parallel with it.

Decay comes first too. A cavity under a bracket cannot be filled without removing the bracket.

Then the space question. If there is not room in the jaw for the teeth in it, room has to be created, and there are three ways: removing a premolar, expanding the arch, or taking a fraction of a millimetre off the contact points of several teeth. Which is right depends on the face, the bite and the amount of crowding. This is the decision people most often are not consulted about, and you will be.

The long middle, and what it is actually like

The first three to five days after the appliance is fitted are the uncomfortable part. The teeth feel tender and bruised, biting into something firm is unpleasant, and ordinary painkillers are enough. That repeats mildly after each adjustment and fades quickly.

After that it is routine: a short appointment every four to eight weeks, a wire changed or the next trays issued, progress checked against the plan. Most of orthodontics is uneventful, which is exactly as it should be.

Cleaning is the part that needs real effort with fixed braces. Plaque left against the enamel at the edge of a bracket demineralises it, and the chalky white square it leaves behind does not go away when the bracket comes off. An interdental brush after every meal prevents it completely. We will size one for you and show you how.

The retainer is not the end of the treatment, it is part of it

Teeth have a memory. The fibres around each one pull it back towards where it started, hard for the first year and gently for the rest of your life.

So the day the appliance comes off, retainers go on. Usually a thin wire bonded behind the front teeth, plus a removable retainer at night. The night-time wear tapers over the years but never quite stops.

Relapse after successful orthodontic treatment is the most common regret in this field, and it is entirely preventable. Anyone who tells you the retainer is optional after six months is telling you what you want to hear.

  • Expect three to five days of tenderness after the appliance is fitted and after each adjustment. Ordinary painkillers are enough.
  • With fixed braces, avoid anything hard or sticky. A broken bracket adds weeks to the treatment.
  • With aligners, wear them twenty two hours a day. Under-wearing is the single commonest reason aligner treatment does not finish on time.
  • Clean around brackets with an interdental brush after every meal. Decalcified white marks around a bracket are permanent.
  • Wear the retainer exactly as instructed, for years, not months. Relapse is the most common regret in orthodontics.
  • Keep your six-monthly cleaning appointments throughout treatment.

Braces & Aligners: questions patients ask

Braces or clear aligners, which should I choose?

It depends far more on your case than on your preference, which is why the records come first. Aligners are excellent for mild to moderate crowding, spacing and relapse after previous treatment, and they are removable, which suits adults who cannot be seen in brackets at work. Fixed braces still handle the difficult movements more predictably, particularly rotating a round tooth, moving a root bodily, or closing a large extraction space. If your case will not finish properly in aligners, we will say so at the assessment rather than after you have paid for them.

How long does treatment take?

Six to twenty four months for most cases, and the honest answer for yours comes after the records are taken. A single instanding tooth or a small relapse can finish in a few months. A crowded case that needs teeth removed to make room takes eighteen months to two years. Nobody can shorten the biological rate at which bone remodels around a moving tooth, so treat any promise of straight teeth in weeks with suspicion, unless what is being offered is veneers, which is a different treatment with a different trade-off.

Am I too old for braces?

No. Teeth move at any age because the bone around them remodels throughout life. Adult treatment does move a little more slowly than treatment in a teenager, and gum health has to be good before any appliance goes on, which is why the assessment includes checking your gums. A very large share of orthodontic patients in Bengaluru now are adults in their thirties and forties.

Will I need teeth removed?

Sometimes, and it is decided from the records, not from a glance. If the jaw simply does not have room for the teeth in it, something has to give, and the alternatives are creating space by removing a premolar, or by expanding, or by slimming a fraction of a millimetre off the contact points of several teeth. Each has trade-offs and the right answer differs by case. You will be shown why before anything is removed.

Do I have to wear a retainer forever?

Effectively, yes, and anyone who tells you otherwise is setting you up for disappointment. Teeth have a memory. The fibres around them pull them back towards where they were, strongly for the first year and gently for the rest of your life. The appliance itself does not, but poor cleaning around it does. It is also the strongest argument for aligners in someone whose cleaning is not reliable, since aligners come out to brush.

Can I get my teeth straightened without braces or aligners?

For a genuinely small gap or a minor chip, composite bonding or veneers change the appearance in one or two appointments without moving anything. That is a legitimate option and we will tell you when it applies. It is not the same as orthodontics, because veneers change the surface of a tooth that stays where it is, and they commit you to maintaining them. Where the teeth themselves are in the wrong place, moving them is the treatment that solves the problem rather than covering it.
