The assessment is the treatment

Whitening gel is a commodity. Anyone can buy it. What you are paying a clinic for is the half hour before it goes on.

That half hour catches the things that turn whitening from a good idea into a bad experience:

• Untreated decay on a front tooth. Peroxide reaching exposed dentine through a cavity is genuinely painful.
• Exposed root surfaces from gum recession. Root is not enamel and does not whiten; it does, however, become very sensitive.
• Cracks in enamel that will let gel in.
• Crowns and composite fillings on front teeth, which will not change colour at all, so your natural teeth lighten around them and the mismatch becomes obvious.

That last one is the most common reason people are unhappy with a whitening result, and it is entirely predictable at the assessment. If you have a crowned front tooth, the honest sequence is whiten first, then replace the crown to the new shade. We will tell you that before you spend anything, not after.

What it costs

We quote a course rather than a session, after the assessment, because a course is what actually gets you a result. Which route suits you is the thing that decides the figure:

• In-clinic whitening. Professional-strength gel applied in timed cycles under gum protection, most of the result in one sitting.
• Take-home trays. Custom-made trays and a lower-strength gel, worn over a week or two. Slower, gentler on sensitive teeth, and easy to top up later.
• Combination. An in-clinic session for the bulk of the change, trays to extend and maintain it.

Internal bleaching for a single dark tooth after a root canal is a different procedure and is quoted separately. So is replacing any existing filling, crown or veneer on a front tooth: those do not lighten with the rest, and once everything around them has changed shade they can end up looking worse than before. We tell you which of yours will need redoing at the assessment, before you start, because for some people that is the thing that changes their mind.

A cleaning first, always

Whitening gel works on the tooth surface, and a tooth coated in tartar and stain is not a tooth surface. So scaling and polishing comes first, in a separate appointment.

For a fair number of patients, this is where the story ends. The cleaning removes enough tea and coffee stain that they decide they do not need whitening at all. We would rather tell you that than take the money.

Managing sensitivity

Cold sensitivity for one to three days is the most common side effect, and it settles by itself. If you already have sensitive teeth, tell us at the assessment. Starting a desensitising toothpaste a week before, and choosing take-home trays over a single high-strength session, makes a substantial difference.

Sensitivity that persists beyond a few days is not normal and is worth a call.
