What happens at your appointment

So you know exactly what to expect before you sit in the chair.

  1. 125 minutes

    Charting and X-rays

    We measure the pocket depth at six points around every tooth and take X-rays. This is what separates reversible gum inflammation from established bone loss, and it is not guesswork.

  2. 245 minutes

    Scaling and root planing

    Ultrasonic and hand instruments clean the root surfaces below the gum line, where a brush cannot reach. Done under local anaesthesia and usually split across quadrants so no single appointment is an ordeal.

  3. 320 minutes

    Healing and reassessment

    Six to eight weeks later the pockets are charted again. In most cases the inflammation has gone, the pockets have shrunk, and this is where treatment ends.

  4. 460 minutes

    Flap surgery, only where pockets remain deep

    If a pocket is still too deep to clean blind, the gum is lifted back under local anaesthesia so the root surface and the bone defect can be seen and cleaned directly, then closed with fine sutures.

    Three-monthly or six-monthly cleanings, depending on how your case behaves. Gum disease is managed rather than cured, and the maintenance is what keeps the result.

The disease nobody comes in for

Almost nobody books an appointment because their gums bleed. They book because a tooth hurts, and the bleeding gums get mentioned on the way out, as an afterthought, usually with the phrase “that is normal for me”.

It is not normal. Healthy gum does not bleed when a toothbrush touches it, any more than healthy skin bleeds when a towel touches it. Bleeding is inflammation, inflammation is a response to bacteria sitting undisturbed at the gum margin, and the process that follows is slow, painless and the single most common reason adults in India lose teeth that had nothing else wrong with them.

The painlessness is the problem. A cavity announces itself. Gum disease removes the bone that holds a tooth in place over ten or fifteen years without ever hurting, and then one day a molar is mobile and it is far too late to do anything useful.

Two stages, and only one of them is reversible

This is the most important thing on this page, so it comes before the treatment.

Gingivitis is inflammation of the gum only. The gums are red, puffy and bleed easily. No bone has been lost. It is completely reversible: a proper cleaning plus a change in daily technique returns the gum to health within a few weeks, with nothing permanently lost.

Periodontitis is what gingivitis becomes when it is left. The inflammation has moved past the gum into the bone, the fibres attaching the tooth to the bone have broken down, and a pocket has formed between the gum and the root. Bone that has gone does not come back. Treatment stops the loss and stabilises what is left, which is genuinely worth having, but it cannot undo what already happened.

Which of the two you have is not a matter of opinion, and it is not something anyone can tell by looking at your gums for five seconds. It is measured.

How we measure it, before we treat anything

At the first appointment we chart pocket depths at six points around every tooth with a graduated probe, and take X-rays that show the level of bone between the teeth.

A healthy sulcus measures one to three millimetres. Four is borderline. Anything at five or above is a pocket that a toothbrush has never cleaned in its life and never will, because the bristles do not go there.

You see the chart, and you see the X-rays. It takes about twenty five minutes, and it is the entire basis for what is recommended afterwards. A clinic that proposes gum surgery without doing this has not measured anything.

Deep cleaning, which is where most cases end

For the majority of patients, treatment is scaling and root planing, and nothing more.

This is not the six-monthly cleaning. That one cleans what is visible. This one cleans the root surface inside the pocket, below the gum, under local anaesthesia, with ultrasonic and hand instruments, removing the hardened deposit and the contaminated layer of root surface that the bacteria are living on. It takes around forty five minutes per quadrant, which is why we usually split it across appointments rather than asking you to sit through the whole mouth in one sitting.

Then we wait. Six to eight weeks later the pockets are charted again, and in most cases the numbers have come down, the bleeding has stopped and the gum has tightened back against the teeth. That is the end of active treatment, and you move onto a maintenance interval.

Flap surgery, for the pockets that do not respond

Some pockets are too deep for any instrument to clean properly without being able to see the surface it is cleaning. If the re-charting shows those, flap surgery is what solves it.

Under local anaesthesia, the gum is lifted back a few millimetres so the root surface and the bone defect underneath are directly visible. They are cleaned under vision rather than blind, any granulation tissue is removed, and where a defect is the right shape a graft material can be placed in it. The gum is then repositioned and closed with fine sutures, which come out after about a week.

It is done at the clinic, under the same local anaesthesia used for a filling, and it takes around an hour per area. The decision to do it is always made from re-charted numbers after the cleaning, never on the first day, because a large number of pockets that look surgical at the first appointment are not surgical at the second.

The laser

We have a soft tissue laser, and it is useful here: less bleeding during soft tissue work, and a more comfortable first few days afterwards than a blade. It is a tool inside the treatment, not a treatment of its own. Nobody should be sold “laser gum treatment” as a substitute for cleaning the root surface, because the cleaning is what works.

Maintenance is the actual treatment

Gum disease is managed, not cured. Someone who has had periodontitis is someone whose gums will do it again if the plaque is left undisturbed, so the interval between cleanings is set from your own re-charted numbers rather than from the standard six months. For some patients that is three-monthly for a year, then longer.

We will also size interdental brushes to your actual gaps and show you how to use them. This is not filler advice. Between-tooth cleaning is where gum disease begins and ends, and a brush sized in a chemist by guesswork is usually the wrong one.

Aftercare

After the appointment

You leave with this written down, and a number you can message if anything feels wrong.

  • Expect some sensitivity to cold for a week or two after deep cleaning. It settles as the gum tightens back.
  • Do not stop brushing the area because it bleeds. The bleeding is the reason to brush it, gently and thoroughly.
  • After flap surgery, no rinsing at all for the first 24 hours, then salt water rinses from day two.
  • Soft food on that side for about a week after surgery, and no smoking, which measurably slows gum healing.
  • Use the interdental brushes we size for you. Floss does not reach into a wide space and a brush does.
  • Keep the reassessment appointment even if everything feels fine. Pocket depth is the only honest measure of whether it worked.

Gum Disease & Flap Surgery: questions patients ask

Are bleeding gums serious?

They are the first sign of the most common reason adults lose healthy teeth, so they are worth taking seriously, but the early stage is entirely reversible. Gingivitis, where only the gum is inflamed, resolves completely with a proper cleaning and better daily technique. Periodontitis, where the bone holding the tooth has started to go, does not reverse. The bone you have lost does not come back. What treatment does is stop it going further, which is why the gap between noticing and being seen matters more here than in almost any other dental condition.

What is flap surgery, and will I need it?

Most people do not. Flap surgery is what we do when a pocket is still too deep after deep cleaning for any instrument to clean it without being able to see. The gum is lifted back a few millimetres under local anaesthesia so the root surface and the bone defect underneath are directly visible, cleaned properly, and then the gum is sutured back. It is done under the same local anaesthesia as a filling, it takes about an hour per area, and the decision to do it is made from re-charted pocket depths six to eight weeks after the cleaning, not on the first day.

Is deep cleaning the same as the regular cleaning I get every six months?

No, and they are often confused. A routine scaling cleans the visible surfaces of the teeth above and just below the gum margin. Scaling and root planing cleans the root surface deep inside the pocket, under local anaesthesia, and it takes about four times as long. If your pockets are healthy you do not need it, and we will not sell it to you.

Will my gums grow back?

Gum that has receded does not climb back up the tooth on its own, and lost bone does not regrow except in specific defects where a graft can be placed. What does happen, and happens reliably, is that the inflammation resolves, the gum tightens against the tooth, the pockets get measurably shallower and the bleeding stops. Teeth that were loose often firm up noticeably. Managing expectations here is part of the job.

Why do my gums bleed if I brush properly?

Usually because the technique is reaching the tooth and not the gum margin, which is where plaque actually causes the problem, or because the spaces between the teeth are never cleaned at all. Brushing cleans about three of the five surfaces of a tooth. The two in between are where gum disease starts, and they need floss or an interdental brush sized for your gaps, which we will size for you rather than leaving you to guess in a chemist.

Can gum disease affect the rest of my health?

Established periodontitis is associated with poorly controlled diabetes in both directions, meaning uncontrolled blood sugar makes gum disease worse and gum disease makes blood sugar harder to control. There is also a body of evidence linking it to cardiovascular disease. We are dentists and will not overstate any of that, but if you are diabetic your gums deserve closer attention than average, and we will tell you that plainly.

Does the treatment hurt?

Deep cleaning is done under local anaesthesia, so the appointment itself is comfortable. Afterwards, expect tenderness and cold sensitivity for a week or two as the gum tightens and a little root surface becomes exposed. Flap surgery is also done under local anaesthesia, with about two weeks of settling afterwards and painkillers for the first two or three days.
