The adjustments are the treatment

The difference between a denture people wear and a denture that lives in a drawer is almost never the denture. It is what happened in the fortnight after it was fitted.

New dentures produce sore spots. That is not a fault; it is unavoidable, because no impression can perfectly predict how gum tissue behaves under load until it is loaded. Those spots take about ten minutes each to find and relieve. Two or three short visits and the denture is comfortable.

What usually happens instead is that the patient is handed the denture, told they will get used to it, endures it for three weeks, and gives up. So at Tooth District the review appointments are booked at the fitting, before you leave, and they are part of the price rather than an extra.

What it costs

We price the options next to each other after the assessment, because the material is most of the figure and the material is your decision to make, not ours:

• Acrylic is the conventional denture: a pink acrylic base carrying the teeth. Cheapest, thickest, and perfectly serviceable. Easy to reline and repair.
• Flexible uses a nylon base that flexes into undercuts, so a partial can be held without visible metal clasps. Thinner and more comfortable, harder to reline.
• Cast partial has a slim cast metal framework with acrylic and teeth on it. Thinnest, strongest, transmits load to the remaining teeth rather than the gum, and lasts the longest. The most expensive of the three, and usually the right answer when you still have healthy teeth to hold onto.

An implant-retained lower denture is quoted separately because it includes implants. It is worth asking about: two implants and a clip attachment solves lower denture lifting, which is the complaint that makes people give up on them.

The lower denture problem, and the fix

An upper denture covers the palate and holds by suction, which works well. A lower denture has a horseshoe of ridge, a tongue moving underneath it and far less surface area, so it holds much less well. Every experienced denture wearer will tell you the lower one is the problem.

Adhesive is a workaround, not a fix. The actual fix is two implants placed in the front of the lower jaw with clip attachments, and the denture snaps onto them. It stays put while you eat. It is a fraction of the cost of a full arch of implants because it is two implants instead of six or eight, and the denture itself does most of the work.

We will raise it at the assessment if your case suits it. Nobody should end up resigned to a lower denture that lifts without having been told this option exists.

What we will tell you honestly

When a denture is the wrong answer. If you have lost one tooth with healthy neighbours, a partial denture is not the treatment; an implant or a bridge is, and a removable plate for a single tooth is a nuisance you will not wear.

When your old denture needs relining rather than remaking. Relining is much cheaper. We are not going to remake something that fits fine at the teeth and only needs its base refitted to a ridge that has shrunk.

What a denture does not do. It restores chewing, appearance and facial support. It does not stop the jaw bone resorbing underneath it. Only implants load the bone in a way that preserves it. That is worth knowing when you compare the prices, because it is the real long-term difference between the two.
